Because we will file a claim to your insurance company as a courtesy to you, we ask that you provide current and accurate insurance information at the time of your visit.  Our surgeons are participating providers for most insurance plans, but you should call your insurance if you would like to verify whether or not we are contracted with your health plan.  If you participate in a plan that requires a referral, you must have your Primary Care Physician or Referring Physician obtain the referral before your visit.  Ultimately, payment for all charges is your responsibility.

If we do not participate with your health plan or you do not have insurance coverage, payment in full will be expected at the time of service.  If you are unable to pay in full, please contact our Business Office prior to your visit so payment plans can be arranged.

Surgery: A deposit is required for all surgical procedures if you have an out of pocket expense due. Payment plans can be arranged prior to surgery with our business office.
